Nurse Practitioner (Part-time)

Radiant Health Centers (RHC) seeks a part-time (up to 20 hours a week) Nurse Practitioner. In this position you will have the opportunity to work with a patient-centered and compassionate healthcare team. The Nurse Practitioner provides advanced sexual health care, prescribes treatments, and serves as a patient’s primary healthcare provider.

With a team of caring and committed individuals and hundreds of dedicated volunteers, we are working to eliminate the stigma and overcome health disparities impacting the Lesbian, Gay, Bisexual, Transgender and Queer + (LGBTQ +) community and to create a healthy Orange County for everyone.

RHC is the largest Orange County provider for such healthcare services and is currently expanding its capacity with plans to create a dedicated Federally Qualified Health Center (FQHC) for the LGBTQ+ community.

The Nurse Practitioner will:

  • Diagnose and/or treat acute healthcare problems such as illnesses, infections, or injuries. Prescribe treatments or therapies.
  • Specimen(s) collection, administration of treatments, performing point of care lab tests.
  • Prescribe medications, counsel patients about drug regimens, possible side-effects orinteractions with other substances.
  • Maintain complete and detailed records of patient’s health care plans and prognoses.
  • Serve as site certifier for the Family Planning, Access Care and Treatment (Family PACT)Program and will be responsible for overseeing the family planning services. We are looking for candidates with the following qualities and qualifications:
  • Minimum 1-2 years’ experience.
  • Master of Science in Nursing.
  • Prefer bilingual (Spanish/English).
  • Nurse Practitioner License and Furnishing Number/License, DEA License, CHDP certified
  • HIV Certification Preferred
  • Knowledge of PrEP and PEP preferred.
  • Experience working and sensitivity towards the LGBTQ + population.

Health Educator/Outreach Worker

Radiant Health Centers (RHC), seeks a Health Educator/Outreach Worker who educates underserved women on the importance of early detection, diagnosis and treatment, and integrated preventive services to prevent and reduce the devastating effects of cancer.

With a team of caring and committed individuals and hundreds of dedicated volunteers, RHC is working to eliminate the stigma and overcome health disparities impacting the Lesbian, Gay, Bisexual, Transgender, Queer + (LGBTQ+) community by providing healthy and compassionate care options, including case management and other services for HIV+ Orange County residents, and medical care for the larger community. The Health Educator/Outreach Worker:

  • Assist in the development and implementation of outreach and recruitment activities of eligible individuals between the ages of 21-64.
  • Coordinate, schedule, and conduct outreach activities at locations where underserved women congregate to assess individual risk behaviors.
  • Distribute relevant information and community resources to participants.
  • Determine eligibility of individuals based on program criteria and continue to follow-up as needed to achieve program outcomes.
  • Support clients by navigating through the process of completing screening exams as part of their routine health care.
  • Document outreach and education activities, and report outcomes to manager.

The ideal candidate will have the following qualities and qualifications:

  • Minimum requirement is a high school diploma or equivalent.
  • Ability to coordinate and handle multiple assignments.
  • Prefer candidate with experience in outreach and community organizing.

Quality Improvement/Compliance Manager

Radiant Health Centers (RHC) seeks a Quality Improvement/Compliance Manager who wants the opportunity to be part of a team who is currently expanding its capacity with plans to create a Federally Qualified Health Center (FQHC) for the LGBTQ+ community. This person will be responsible for the oversight of clinic analytics to ensure a proactive approach to the improvement of quality care, patient outcomes and maximization of resources.

With a team of caring and committed individuals and hundreds of dedicated volunteers, RHC is working to eliminate the stigma and overcome health disparities impacting the Lesbian, Gay, Bisexual, Transgender, Queer + (LGBTQ+) community by providing healthy and compassionate care options, including case management and other services for HIV+ Orange County residents, and medical care for the larger community.

The Quality Improvement/Compliance Manager’s role is to:

  • Plan, design, implement, evaluate, and maintain a quality improvement program and compliance plan.
  • Lead the development and management of a clinical analysis program to support clinic management in the decision-making process related to clinical quality improvement.
  • Responsible for developing methods for data collection and extraction.
  • In collaboration with the Medical Director, ensure QI plan meets clinical standards of care and achieves the desired outcomes.
  • Coordinate the tracking and reporting of clinical outcomes and recommend a corrective action plan.
  • Revise policies and procedures annually to ensure compliance with legal, risk management, accreditation, and regulations.
  • Monitor clinic for compliance with OSHA, Infection Control, CLIA and Material Data Sheets (MSDS) standards.

The ideal candidate will have the following qualities and qualifications:

  • Minimum requirement is a Bachelor of Science in Health Science, Administration, or related field.
  • Minimum of 3- 5 years of progressive experience in Quality Improvement, Compliance, and coordination of healthcare programs in a healthcare facility, preferably with a federally funded health center.
  • Possesses knowledge of clinical quality principles, methods, and tools, including computer application related to QI.
  • Demonstrated understanding of the primary care practice management environment.
  • Must be fully vaccinated and boosted.

Medical Assistant

Radiant Health Centers (RHC) seeks a Medical Assistant for our medical clinic. This position provides a professional front office environment for our patients. Supports medical staff with back office administrative and organizational tasks as well as examinations. Helps to ensure a streamlined and efficient patient workflow in the clinic.

Medical Assistant will:

  • Schedule and confirm medical appointments.
  • Keep patient appointments on schedule by notifying provider of patient’s arrival; reviewing service delivery compared to schedule; reminding provider of service delays.
  • Take patient medical histories and vital signs.
  • Ensure patient has completed all required paperwork prior to seeing the Provider.
  • Follow up on patient visits as instructed by Provider.
  • Update patient charts.
  • Administer topical, oral or intramuscular medications.
  • Perform basic laboratory tests.
  • Assist Provider with examinations.
  • Call in or send prescriptions to pharmacies.

The ideal candidate will have the following qualities and qualifications:

  • Minimum requirement is a High School Diploma, GED or equivalent experience.
  • Medical Assistant Certificate from an accredited Medical Assistant program required.
  • Current CPR Certificate required.

Service Coordinator

Radiant Health Centers (RHC) is seeking a full-time Services Coordinator whose role is to assist clients with access to eligible services. This position requires an individual who can multi-task and remain organized in a fast-paced environment.

The Service Coordinator role is to:

  • Assess and pre-register potential new/returning clients to determine immediate needs.
  • Field client calls and walk-ins from clients requesting services, making referrals as appropriate.
  • Coordinate services and referrals for clients who have reached a higher level of independence.
  • Complete and maintain up to date RHC pantry authorizations, follow-up with clients, needing eligibility screening.
  • Assist program coordinators with administrative tasks, submit documentation requests, request physician authorizations, follow-up with clients needing eligibility screening.
  • Translate and interpret for program coordinators with monolingual Spanish speaking clients.
  • Follow up with clients on case management needs as directed by case managers.

The ideal candidate will have the following qualities and qualifications:

  • Minimum of a high school diploma or comparable case management experience. A minimum of 2-4 years case management experience is preferred.
  • The ability to multi-task and maintain a high level of organization.
  • Must demonstrate a positive and caring attitude, be client focused, and value and respect others.
  • Must demonstrate professional phone etiquette
  • Community outreach experience preferred.

PreP Navigator

Radiant Health Centers (RHC) seeks a PrEP (Pre-Exposure Prophylaxis) Navigator who provides support, education and guidance to individuals who require assistance in accessing and remaining in PrEP services. In addition, this position supports community outreach efforts to increase awareness of PrEP to priority populations.

RHC is the largest Orange County provider for such healthcare services and is currently expanding its capacity with plans to create a dedicated Federally Qualified Health Center (FQHC) for the LGBTQ+ community.

Job Requirements:

  • Provide information regarding PrEP and offer PrEP mavigation services to all clients who test at RHC.
  • Assist client with enrolling in any supplemental programs that cover PrEP treatment and related expenses.
  • Provide comprehensive education about PrEP; it’s purpose, walkthrough the treatment process, provide details of follow-up visits, and explanation of retention guidelines.
  • Provide HIV education including sexual health.
  • Participate in the development and implementation of training for Case Managers and Mental Health staff.

The ideal candidate will have the following qualities and qualifications:

  •  Minimum requirement is a high school diploma or equivalent.
  • Specialized training or experience in HIV prevention interventions.
  • Have a current HIV test counselor certification provided by the California of Public Health, or obtain within 30 days of employment
  • Must have experience working with youth and persons at high risk of HIV infection.
  • Bilingual (English and Spanish) preferred.
