Director of Clinical and Case Management Services

Radiant Health Centers (RHC), seeks a Director of Clinical and Case Management Services who has proven leadership capabilities to drive departmental expansion, manage and analyze data to inform program decisions, strong budgetary experience and key management experience in leading teams and workgroups.

With a team of caring and committed individuals and hundreds of dedicated volunteers, RHC is working to eliminate the stigma and overcome health disparities impacting the Lesbian, Gay, Bisexual, Transgender, Queer + (LGBTQ+) community by providing healthy and compassionate care options.

RHC is the largest Orange County provider for such healthcare services and is currently expanding its capacity with plans to create a dedicated Federally Qualified Health Center (FQHC) for the LGBTQ+ community.

The Director of Clinical and Case Management Services will:

  • Create departmental strategic plans then translate into program action plans. Provide leadership for the development, implementation, management and evaluation of plans
  • Provide exemplary, innovative, collaborative, and dynamic leadership, and support team participation throughout the organization
  • Seek, administer and monitor grants and assist Chief Program Officer by developing and implementing budgets
  • In conjunction with Quality Assurance staff, responsible for quality assurance for required documentation. Ensures proper handling and transfer of documents and records
  • Utilize data to track and report program performance and progress and identify areas of improvement to promote organizational growth

The ideal candidate will have the following qualities and qualifications:

  • Must have experience working with individuals who have experienced homelessness, who have traumatic histories, identify as LGBTQ+, are gender non-conforming and/or who often face multiple issues and have complex needs.
  • Excellent project management skills, ability to balance multiple priorities with proven ability to anticipate challenges, solve problems and maintain a calm demeanor in high-pressure situations
  • Strong budget management experience
  • Proven ability to drive the expansion of a department
  • Experience training, mentoring, and supervising staff and teams with the proven ability to foster a healthy, balanced and accountable environment for staff
  • Four years of leadership experience with increasing responsibility of a comparable team size preferred
  • Ability to provide clinical supervision (Strongly preferred)
  • Master of Social Work, Counseling or related degree required (LCSW or LMFT strongly preferred)

Director of Individual Giving

Radiant Health Centers (RHC), seeks a Director of Individual Giving who will collaborate with the Chief Development Officer to build and implement a comprehensive, proactive strategic plan focused on expanding and enhancing the agency’s individual donor base and help to ensure the achievement of performance and revenue goals.

The Director of Individual Giving will:

  • Manage all aspects of donor relationships including identification, research, qualification, cultivation, and stewardship to foster a portfolio of donors
  • Provide leadership and direction for donor outreach and pipeline building programs tailored toward engagement
  • Produce timely routine reports used to drive donor strategies
  • Ensure fulfillment of donation and subsequent recognition

The ideal candidate will have the following qualities and qualifications:

  • Demonstrate leadership qualities and exceptional relationship-building skills
  • Ability to influence, work pro-actively and effectively to build consensus among cross-functional teams and external partners.
  • Working knowledge of Microsoft Office and database knowledge of donor CRM software desirable(Raiser’s Edge, Donor Quest, Salesforce) and prospect research tools
  • Must be highly organized, self-motivated, professional, able to manage multiple projects under tight deadlines
  • Proven track record in securing gifts and producing results
  • Minimum five years of comprehensive development experience including directing annual giving and annual campaigns, individual giving and/or membership societies
  • Bachelor’s Degree from an accredited institution in Business, Marketing, Communications/PR, Non-Profit Management or related fields.

Program Quality Assurance and Improvement Administrator

Radiant Health Centers (RHC), seeks a Program Quality Assurance and Improvement Administrator
to oversee quality assurance, improvement, and compliance functions using a data-driven approach. Ensures programs and services are implemented at the highest standards and clients receive the highest of care.

Scope of work includes but is not limited to:

  • Quality management oversight of public and private grants
  • Measure and evaluate data to determine the impact of the programs on both the operational outcomes and quality client care
  • Represent agency on internal and external Quality Management Committees
  • Audit programs using established protocols as required by contractual agreement or agency standards
  • Prepare and submit required quality management reports on a monthly, quarterly, and annual basis as specified by each contract/grant
  • Collect and maintain data that is required for a specific contract.

We are looking for a Program Quality Assurance and Improvement Administrator with the following qualities:

  • Possesses superior attention to detail; quality and precision focused
  • Previous experience in customer service with strong communication skills
  • Must have excellent analytical thinking skills and problem-solving abilities
  • Previous experience as a project manager, or coordinating large projects is an asset
  • Bachelor’s degree preferred or equivalent experience

Case Manager Assistant – Part-time

Radiant Health Centers (RHC), seeks a part-time (20 hours a week) Case Manager Assistant who is responsible for assisting with a wide variety of case management functions.

With a team of caring and committed individuals and hundreds of dedicated volunteers, RHC is working to eliminate the stigma and overcome health disparities impacting the Lesbian, Gay, Bisexual, Transgender, Queer + (LGBTQ+) community by providing healthy and compassionate care options.

RHC is the largest Orange County provider for such healthcare services and is currently expanding its capacity with plans to create a dedicated Federally Qualified Health Center (FQHC) for the LGBTQ+ community.

The Case Manager Assistant will:

  • Translate and interpret for case management staff with monolingual Spanish speaking clients.
  • Follow up with clients on case management needs as directed by Case Managers and assist with Case Manager of the day calls.
  • Create charts and organize closed client charts.
  • Assist Case Managers with administrative tasks such as fill the cabinet with forms, order forms, take minutes for clinical staff meetings, create Case Managers work schedules, update physician lists, order supplies, and schedule meetings,
  • Assist with audits.
  • Arrange mental health and medication co-pays.
  • Request labs from doctors and verification of disease (VOD), order durable medical equipment (DME) and certificate of Eligibility Waivers.
  • Liaison between Lifeline and RHC.
  • Assist the Case Manager of the day with calls.
  • Train new Social Workers, Case Managers and Nurses.

Working for a non-profit requires a unique and dedicated individual. We are looking for candidates with the following qualifications:

  • Bilingual (English/Spanish) Required
  • High School diploma or G.E.D preferred. Comparable experience is acceptable in lieu of a diploma.
  • Ability to follow directions and instructions.
  • Organizational abilities and the ability to file alphabetically.
  • Must demonstrate professional telephone etiquette and the ability to interact with a diverse group of people.
  • Individual must demonstrate good judgment.
  • Must have the ability to work with diverse clients, volunteers, and employee populations.
  • Must demonstrate a positive and caring attitude, be client-focused; value and respect others, demonstrate ethical behavior, teamwork and collaboration.
  • Must possess sensitivity to the needs of individuals impacted by the HIV disease.
