Clinical Nurse Case Manager Lead

Radiant Health Centers (RHC) seeks devoted, compassionate, Clinical Nurse Case Manager Lead to coordinate whole-person care for all clinic patients, specifically a lead who can manage a team of case managers. The nurse case manager lead will participate in and lead a team to review patient charts, meet with providers, patients and their families to establish the most suitable course of action. The candidate can quickly identify and remove burdens from patients to ensure they are afforded the time and space to focus on their health. Case management activities incorporate a collaborative, interdisciplinary team approach. Ultimately, a remarkable Clinical Nurse Case Manager Lead will establish warm and meaningful ties with each patient their families, and bring together a cohesive, efficient, and effective case management team.

Job Requirements:

  • Conducting intake assessments with patients and families.
  • Obtain and maintain necessary documentation regarding client’s medical and demographic data, including but not limited to, necessary consents.
  • Develop, implement, monitor, and modify a service care plan through an interdisciplinary team process that account for physical, financial, and other constraints. Routinely review and revise service care plans.
  • Monitor all services provided to the patient and assure that all available resources are being utilized.
  • Evaluate and anticipate patient needs, provide support, maintain role as patient liaison while fostering independence and decision making on the part of the patient.
  • Provide home visits to patients as needed.
  • Based on client assessed needs, make appropriate referrals, e.g., to a health care provider, social worker, benefits counselor, or spiritual counselor.
  • As lead, participate in ongoing program evaluation, utilization review and quality assurance activities as directed by the immediate supervisor.

The ideal candidate will have the following qualities and qualifications:

  • Must possess a Registered Nurse license.
  • Two (2) years’ experience in clinical nursing preferred.
  • Experience in the LGBTQ+ community is also a plus.
  • Supervisory experience.
  • Bilingual (English and Spanish) preferred.
  • Must be fully vaccinated and boosted.

Medical Assistant

Radiant Health Centers (RHC) seeks a Medical Assistant for our medical clinic. This position provides a professional front office environment for our patients. Supports medical staff with back office administrative and organizational tasks as well as examinations. Helps to ensure a streamlined and efficient patient workflow in the clinic.

Medical Assistant will:

  • Schedule and confirm medical appointments.
  • Keep patient appointments on schedule by notifying provider of patient’s arrival; reviewing service delivery compared to schedule; reminding provider of service delays.
  • Take patient medical histories and vital signs.
  • Ensure patient has completed all required paperwork prior to seeing the Provider.
  • Follow up on patient visits as instructed by Provider.
  • Update patient charts.
  • Administer topical, oral or intramuscular medications.
  • Perform basic laboratory tests.
  • Assist Provider with examinations.
  • Call in or send prescriptions to pharmacies.

The ideal candidate will have the following qualities and qualifications:

  • Minimum requirement is a High School Diploma, GED or equivalent experience.
  • Medical Assistant Certificate from an accredited Medical Assistant program required.
  • Current CPR Certificate required.
  • Bilingual (English and Spanish) required.

The Director of Programs leads and directs Programs within HIV Care Services.  The Director implements and evaluates programs with emphasis on measurable outcomes using data and best practices; identification, development and management of financial resources to support programmatic priorities, effective management and engagement of staff to achieve departmental goals, provide quality services and promote shared accountability.

  • Ensure ongoing excellence that all programs operate consistently and ethically within the mission and core values of the agency.

We are looking for candidates with the following qualities:

  • Licensed Clinical Social Worker – required.
  • Knowledge and understanding of psychosocial issues of HIV infected individuals.
  • 6-8 years of professional experience in managing programs including direct experience working in or with individuals who have experienced homelessness, who have traumatic histories, identify as LGBTQ, are gender non-conforming and/or who often face multiple issues and have complex needs.
  • Knowledge of community resources such as Orange County Social Services, Social Security, as well as other private non-profit social services agencies.
  • Proven leadership and success in engagement and mobilization at the individual, community, and organizational levels, strong interpersonal and relationship building skills, with the demonstrated ability to influence others.
  • Demonstrated ability to manage to an outcomes-based approach; proven experience of outreach and mobilization, managing outcomes, and developing and using data to inform continuous improvement and decision-making.

Social Work Case Manager  

Radiant Health Centers (RHC) is seeking a full-time (40 hours a week) Social Work Case Manager. The

Social Work Case Manager provides psychosocial support, and direct clinical practice with HIV+ clients and client systems. Many Social Work clients struggle with mental illness, substance use, homelessness, and complex medical issues. Candidate should have knowledge and understanding of psychosocial issues of HIV infected individuals, and strong background in resource development and client advocacy.

  • Assess and supervise a primary caseload.

  • Assist in the development of agency programs to address psychosocial needs of clients.

The ideal candidate will have the following qualities and qualifications:

  • Candidate must have a Master of Social Work, MFT or equivalent. A licensed Social Worker is preferred.
  • Outstanding communication and organizational skills.
  • Ability to work with diverse populations, be sensitive to their needs, ability to be assertive.
  • Community outreach experience preferred.
  • Ability to evaluate and assess clients need and develop a sense of trust and rapport.
  • Bilingual (English and Spanish) preferred.
